Visitors moving to the area

Visitors moving into the area are welcome to register at this surgery. 

Please note that we only receive patients who are entitled to receive NHS treatment and whose permanent address lies within our practice catchment area (click here)

 

 
Registering as a patient

You can register at the practice by bringing your medical card to the surgery.

If you do not have a medical card you will be asked to complete a registration form (GMS1) giving your name, address, date of birth, and details of your previous address and doctor.  This allows the Health Authority to arrange for your medical record to be transferred to us.  A copy of this form can be downloaded by clicking here.

As it will take a little while for your medical records to be forwarded to us from your previous GP, it would be helfpul if you would complete this health questionnaire and submit it alongside your regsitration form.

If you cannot come to the surgery, registration may be may be carried out by an authorised representative.

 

 
Doctors' lists

Our reception staff will inform you which doctors' lists are open and you may express a preference for registering with a particular doctor.

We do our best to accommodate your wishes but please be aware that we cannot register you with a doctor whose list is closed.

We do encourage families to use the same doctor where possible. We feel this to be in your best interest.
