It is not possible to provide all services under the National Health Service and the following table therefore shows the current level of private fees, and the services for which you may have to pay.

If you have any concerns about the level of fee involved, please discuss this with your doctor prior to requesting the service.

NB: If you require a Yellow Fever vaccination, and are not a registered patient at the Glastonbury Surgery, you will need to complete a form which can be downloaded by clicking here.  Your GP will need to complete part of the form, so please remember to allow sufficient time for them to do this, and for you to collect it prior to your appointment with us.  If you do not have a completed form with you at the time of your appointment, we will not be able to provide you with the vaccination.
