You can register at the practice by bringing your medical card to the surgery. If you do not have one you will be asked to complete a registration form (GMS1) giving your name, address, date of birth and details of your previous address and doctor. This allows the Health Authority to arrange for your medical records to be transferred to us.
If you cannot come to the surgery registration may be may be carried out by an authorised representative who may sign the registration on your behalf.
When you register with the practice you will be asked to complete our registration documents. This allows for your current information and family history to be recorded prior to the receipt of your medical records.
