Travel health and vaccinations

Please download this travel form, or collect one from the surgery reception, and complete it as fully as possible.

Please also take the time to visit www.fitfortravel.nhs.uk to read up about Hepatitis B, rabies, and other risk factors that may be relevant to your travels, so that when you see the nurse you are well informed.

Please then book an appointment with a nurse and bring your completed form with you.  If you do not being your completed form with you we cannot address your travel needs safely in the allotted appointment time and may have to send you away to rebook.

We recommend you book your appointment a minimum of 6 weeks before you are due to travel, as some vaccines are given as courses over weeks.

The surgery does not have last minutes appointments available, and any travel vaccination should be given a minimum of two weeks before you go, so please plan your trip to the surgery well in advance.  If you book in with a nurse too close to your travel date we may not be able to offer you vaccines.  Alternatively, you could visit a travel clinic.

At this initial appointment the nurse will discuss your requirements.  You may not receive any vaccinations at this first appointment, but, if necessary, appointments will then be made for your vaccines.

If you are going on an extended, complex trip visiting several countries, please book a double appointment initially.
